Primary Care RVU Calculator - Frequently Asked Questions

What is the most commonly billed primary care CPT code?

CPT 99213 (established patient, low complexity) is the highest-volume primary care code nationally. Under the 2021 E&M revisions, 99213 carries 1.30 wRVU. At the 2026 CF of $33.40, the estimated Medicare payment for a non-facility 99213 is approximately $89 (2.69 total RVU x $33.40).

How does time-based billing work for primary care E&M codes?

Since 2021, physicians can select E&M level based on total time on the date of the encounter. Total time includes face-to-face time plus pre- and post-service work. Thresholds: 99212 = 10-19 minutes; 99213 = 20-29 minutes; 99214 = 30-39 minutes; 99215 = 40-54 minutes. For 55+ minutes, bill 99215 plus prolonged services add-on (99417 or G2212).

What is the difference between a preventive medicine visit and an Annual Wellness Visit?

A preventive medicine visit (99381-99397) is a comprehensive physical exam covered by commercial insurance. The Medicare Annual Wellness Visit (G0438 for first visit, G0439 for subsequent) focuses on health risk assessment, a personalized prevention plan, cognitive assessment, and advance care planning. AWVs are free for Medicare beneficiaries and cannot include comprehensive physical examination components.

How are Transitions of Care Management codes used in primary care?

TCM codes (99495, 99496) are billed when primary care physicians contact a patient within 2 business days of discharge and see the patient within 7-14 days. CPT 99495 (moderate complexity TCM, within 14 days) carries 2.78 wRVU. CPT 99496 (high complexity TCM, within 7 days) carries 3.79 wRVU - significantly higher wRVU than standard E&M codes.
